Maintenance & Treatment Membership 

Cancelation Policies

 

A minimum of 30-day’s notice is given in advance of your monthly dues date. Written notice is to be sent to AmbianceMedSpa@gmail.com. As of the effective date of a cancellation, any unused rollovers expire 30 days after cancelation date. 

 

Memberships are a minimum of three months. Membership cancellations must have a 30-day advance written notification. Reminder, any pending membership dues within the 30-day period will be charged and credited to member services to be used within a 30-day period after cancelation.

 

 If you cancel prior to the end of your terms, (3month period) you will be billed a $50 cancellation fee in addition to the pro-rated fees. There are no refunds on services and cannot be redeemed for product. 

 

Exceptions are as followed:

You may cancel your contract in the middle of your series on the following conditions:

*You move more than 25 miles away, with proof of address provided to management. (utility bill preferred)   

*You have a permanent disability with a written note from your doctor. Once you have been approved for cancellation, your membership will be cancelled, effective immediately

Ambiance Medi Spa reserves the right to modify the terms and conditions of the membership program upon advance prior written notice.
